This dog tag has a smooth edge and can fit on up to a 3 mm chain. Text engrave the front with up to 3 lines (+$10 per line) and the back with up to 4 lines (+$10 per line) to personalize with your important emergency medical information. This dog tag measures 1 1/2 inch (L) x 7/8 inch (W), with a 1 mm thickness.

How to: Engrave your tag
1. Tag Front Engraving
Engrave your most important identification information here. Name, primary medical condition and In Case of Emergency (ICE) contact number.
2. Tag Back Engraving
Engrave using upper case lettering for clarity and legibility. List restricted medications, medication allergies, chronic medical conditions, blood type and recent surgeries.
3. Tag Back Engraving
List any food allergies, and if you carry an EpiPen. Also detail any other medical conditions and any other ICE numbers such as a Doctor or guardian.
How to Engrave your Medical Dog Tag
